I have run into a number of quite irritating bugs during my most recent playthrough, three of them are annoying while one of them is what I consider to be serious, due to it rendering a particular weapon unusable.
Bug 1: The Talon shotgun-pistol cannot be used by AI team mates. What I mean by this, is that they don't actually shoot it when equipped. They will come out of cover while aiming at their target, but they just stand there refusing to fire. Liara even shakes as if she is firing a submachinegun. Sometimes Tali will not even come out of cover for an entire battle.
Bug 2: The quest, Ismar Frontier: Prototype Components, yields no reward at all.
Bug 3: On one playthrough, I was not given the chance to give Ashley the gift that I brought at the Sirta Foundation kiosk. The dialogue option didn't even start, it just went straight into quizzing me about having cheated on her in the previous game.
Bug 4: Towards the end of the game, when Hacket is given his speech just before we assault Reaper controlled Earth, he says something about the Crucible, however he is cut short by a scene transition mid sentence. It did this only after the Extended Cut was installed.
