I'm going through Insanity as a Sentinel and I was wondering what would be the best way to approach it in terms of what armor to use, what bonus power to have and what squadmates to bring along and what weapons THEY should have.

Overload shields and barriers.
Then warp (or any other biotic primer from squadmates) - throw armor and health.
The rest doesn't matter.
Run the armor you think looks cooler, the squadmates with interesting dialogs and weapons you find fun to use.
PS: If you really want to make the game boring pick Kaidan and Javik, give them a particle rifle (another one for you) and choose squad AP ammo as your bonus power.

When i did my insanity run on my sentinel, I was comfortable enough using the following on insanity, as it pertains to your question:
- blood dragon armor
- warp ammo for bonus power
- squadmates: garrus for sure cuz he's my boy, and switched between javik and kaidan/liara just for sh*ts and grins, but preferred javik as 2nd squaddie. I know some people out there prefer EDI..to each their own.
- weapons for squadmates were vindicator (assault rifle) and widow (sniper rifle) for garrus, particle rifle (AR) for javik (and best Pistol you have). For Liara, locust (SMG) and paladin (spectre weapon - heavy pistol) for her, and kaidan vindicator (assault rifle) and Paladin.
Insanity in ME3 is really easy compared to ME2, which I am sure you have heard people talking about to no end, so you can "get away with" experimenting with certain squad members to see which compliments your play style best. This is also what can be fun about ME3... You don't feel like you ar "stuck" with the same group, but that you can really play around and bring different people and not shoot yourself in the foot. As mentioned in the previous post, you get different dialogue and such which makes the game fun.
The above are just my suggestions because i've played a sentinel extensively and this is what worked for me. However, my favorite class is the adept
The parts of the game that were even remotely difficult on insanity were both the kai leng fight and the very end of the last mission (priority: earth 2). But these were more fight mechanics issues rather than who I brought on the mission and what my gear/bonus power was.
The sentinel is a great class to play on insanity and you will do great! Have fun!

Double throw + 2 biotic squadmates = biotic explosions
+ warp ammo
squadmates should be liara/javik (tempest + paladin / particle rifle + paladin) never let liara use smg. only pistol.
Doing this, makes insanity seem like casual difficulty.

SMGs have horrible effective range, and Liara likes to stay at distance. She will put more accurate damage down range using a pistol.

Yea forgot to mention that in my post...smg's are bad in ME3, i agree with the above posters? You have to equip the smg on liara, but you dont have to let her use it

that said, using an N7 Hurricane on Shepard can work. Get up close and fire from the hip. It will rip things apart. The hornet is also pretty good, however, like all SMGs you pretty much have to fire it from the hip. The recoil is too much on aimed shots.

Odd. I sometimes use the hurricane on Liara with
squad cryo while on an infiltrator. I don't really pay attention
to her weapon dps, she is there for biotic explosion
combos and stasis bubbles. But with the hurricane,
she's certainly freezing things all the time. Pistols
are also good, but the hurricane sounds better!

I'm going through on Insanity atm as an Adept.
I've found guns to be mostly useless, but Biotics take enemies apart relatively easy.
Javik and Liara pretty much all the time.
Liara's Stasis + my Warp = lots of dead bad guys

Ridiculously easy to you after that especially if you are using an imported character. Pause game with power wheel + radar = god mode.

Seriously though get overload crowd control, recharge, shield damage, double throw. Bring javik with double pull (ignore slam, max dark channel only after pull and passive). Use a paladin on liara and yourself with a scope. GIve javik particle rifle with extended mag or paladin. Scope is nice for sniping. I never bothered with smgs. Keep power recharge at 200%. always take detonate evolutions on powers. Get squad warp ammo with liara.
Bonus power: defense matrix. Max tech armor for power damage and recharge. Def matrix for tech damage. Purge the matrix when you need to refill your shields and run for cover.
Playstyle is overload shield, pull or singularity, throw for explosion. Repeat. Throw recharges really fast so if javik is on cooldown you can use liara to prime enemies for explosions and vice versa. Armored enemies are primed with liaras warp or javiks dark channel. Health enemies are primed with pull or singularity. Stasis is helpful against phantoms and centurions. You can use stasis on them and detonate the stasis while they are still shielded. Lift grenades are very good vs boss enemies. Between you and javik thats 12 grenades!!! Dont be afraid to use them. The big ammo crates refill 2 grenades. So you can refill them during missions. Better to use all the grenades than not use any.
Hardest spots are kai leng, primes ln rannoch, priority earth.
For the missions you use tali max sabotage for tech vulnerability and duration. Only max one drone power you cant use both at once.
