Now I know we've talked about this to death and I know the prerequisites. However, just finished my insanity playthrough and... nothing. I set it to insanity from square one and didn't touch it. HOWEVER, once, being the fidgety bastard that I am, I flicked through the difficulties while playing but pressed b and checked back to make sure it was still on Insanity.
Doubt that would have affected the achievement, but still. Anyone? Suggestions? Sympathetic pats on the head?

Did you make sure to do every mission? All the N7 Missions? Meet up with all the sidelined ME2 Characters?
When I played Insanity and got the achievement, I did everything. Every War Asset from the Galaxy Map, everything.
I wouldn't be surprised if that flick thing is what screwed you over....And I feel bad for you because 3 Banshees, 8 Brutes, and endless everything else was a pain in the ass and took forever to do....

I would say that even just looking at the other difficultys may null the acheavment idk as i never did that during my playthrough as I was Paranoid that what happend to you was gonna happen

I went to that screen once after setting, but did not flick or change anything. So hopefully it works.
The insanity achievement is "complete without changing difficulty after leaving earth". If you flicked, that could be counted I suppose.
And note the two insanity achievements. One for completing the storyline on insanity, and one for all missions on insanity (eg: You have to do all the N7 and side missions.

Wow, as it turns out, the ending you choose actually matters. I decided to say no to all the endings to see what the result would be and that's why I didn't get anything. So, in conclusion, only sysnthesis, control or destroy will get you the achievements. Saying no is consodered not finishing the game. I got both achievements.

That is interesting. Since "choosing no" was not an original ending option, it probably does not trigger the Insanity achievements like the other endings do.GardenSnake wrote...
