Fireteam. Miranda.
Hacker. Legion/kasumi (tried both still having same problem.
Biotic. Smaria/Jack
Escort: Grunt.
Final Fight i bring mordan and tali with me.
Each time glass tubes open i can 2 in one shot so it only takes me 2 tube openings.
I don't get it no matter what i do someone gets pinged by a stray bullet and i end up with one dead. Everyone is loyal and i even had enough paragon to stop both confrentations. I want everyone to live so i have a better story in ME3.

Final Fight: As an Adept, I brought Mordin and Thane with me. Otherwise it usually is Garrus/Miranda.
This way they always survive

Garrus, Grunt and Zaaed are good
Kasumi, Tali, Mordin and Jack are bad
Taking Mordin and Tali with you is good
sending Grunt on escort while leaving Jack and Ksumi present is bad
You don't mention whether you have Garrus or Zaeed with you, one of them holding the line should balance out Jack and Kasumi's weakness making it touch and go (both should make it easy).
Ultimately sending Jack or Kasumi as escort while leaving Grunt to hold the line (and keeping Mordin and Tali for the final fight) should be safe

I take Samara and Thane with me to the Human Reaper and always have all 12 of my team mates survive as well as all my crew.

I'm not sure which part your team is getting killed from your description, but:
My Fire Team leaders are Garrus and Miranda
Hacker is Legion
Biotic is Samara or Jack
Escort is Mordin (he can easily die in the hold the line bit, and will be the first if he does)
Try to leave some "strong" members like Zaeed, Grunt etc at the hold the line bit as this makes it more likely the team will survive without casualties. So your team for the Human Reaper should be someone generally weak, with not alot of firepower but maybe some useful powers ( Warp, combat drone etc)
That should make the whole team survive.

This is the only thing I can think of but it still doesn't make sense. I took Grunt (with Miranda) to the final battle in my first playthorough so he wasn't counted in holding the line, yet everyone lived. Garrus and Zaeed should be enough to ensure no one dies if everyone is loyal.
Sorry to ask this but is Zaeed definitely loyal? It's possible to finish his mission using the paragon path and not get his loyalty. Worth double checking I guess...

You can choose the paragon path to save the refinery workers and still get Zaeed's loyalty if your paragon ratio is high enough. It's often greyed out later in the game if you've missed too many other paragon points but it's very achievable if you do his mission early.sweet-d wrote...
Yeah you have to do the renegade option to get Zaeed loyal. But he still lived on mine when I chose the paragon option. But it does come back to get you later if you choose paragon for Zaeed.
However, all this is irrelevant. Zaeed's loyalty, or even his presence (the OP doesn't say whether or not they have Zaeed in the first place) doesn't change things if everyone really is loyal. If you calculate it for loyal Zaeed, unloyal Zaeed and no Zaeed using the chart Quetzal01 linked to before, and with Grunt, Mordin and Tali all off doing other things, Kasumi being there, and assuming everyone is loyal you get:
4 - Zaeed
4 - Garrus
2 - Thane
2 - Legion
2 - Samara
2 - Jacob
2 - Miranda
1 - Jack
1 - Kasumi
20 - total, and 20 ÷ 9 is 2.222r which means everyone will survive. Even if Zaeed is not loyal his score only drops to 3, and 19 ÷ 9 is still just over 2 and means that again everyone will survive. But without Zaeed at all...
4 - Garrus
2 - Thane
2 - Legion
2 - Samara
2 - Jacob
2 - Miranda
1 - Jack
1 - Kasumi
16 - total, and 16 ÷ 8 (one less character present) is exactly 2, which again means survival for everyone, though only just. Assuming that all the info on the maths behind surviving Hold The Line is accurate it's really not clear why the OP is having a casualty each time. For there to be one casualty the combined score should be no higher than 17 with a 9 person team and 15 with an 8 person team. Tends to suggest that at least two team members, possibly more, are not loyal after all. Or I've totally misunderstood how the HTL scoring works and got lucky in my own playthroughs.
OP, if you're reading, in the team screen does everyone have the red circle underneath to show that they're loyal? Do you have the Gibbed Save Editor and if so have you used it to double check that all loyalty flags are checked? If yes then I think more detail may be needed. Exactly who does what job, including the names of all the Hold The Line team, and who dies, from what and at what stage?

Each squadmember has a certain point value, and if I remember correctly, Grunt, Garrus, and Zaeed have the highest, so removing Grunt probably wouldn't be something I'd advise against.
