Just very plain and simple:

Incendiary Ammo 4: Squad Incendiary Ammo
Charge 4: Heavy Charge (in yo face!)
Shockwave 4: Improved Shockwave
Pull 1
Assault Mastery: Guardian
Reave 4: Area Reave

Weapon Training: Sniper Rifle, for those bastards far away and up on ledges you can't charge.

Only thing I wonder about is team. Garrus/Thane/Zaeed for sniper + Miranda for biotics seems best to me, so we have close + mid + ranged covered.

Thoughts?

That build should be fine. It depends on how much charging you want to do or end up doing. If you end up charging the whole game, Reave becomes a power you never use and you are better off putting the points into (Squad) Cryo Ammo.

I like using a sniper rifle (Viper) on my Vanguard, but only if I do not take the Claymore. Most Vanguards I have seen on these forums use either the Claymore or Viper. Note that by the time you get your bonus weapon (unless you mod) there are just not that many unchargeable enemies left. But a sniper rifle is still good if you are uncomfortable charging or just do not feel like charging constantly.

As for squadmates, it does not really matter a whole lot. Everyone works pretty good in some capacity with the Vanguard.

I kinda do 50/50 charging/softening up from distance, so hell yeah reave and hell yeah sniper :D

I never seemed to use reave a whole ton. For ranged Viper is a beast for taking down armor, but then again so is Carnifax. Vanguards are nice in that you have a lot of options that will all work fine. The downside is you'll probably never use them because your just gonna be charging all over like a bat outta hell.

Claymore is obviously epic but to me it is so unforgiving. I remember getting frustrated and just using eviserator anyways so I might take the sniper route just for the heck of it. Still if you like high risk high reward....there is nothing quite so satisfying as a claymore blast to the dome.

For companions you have a lot of options. Anyone with a sniper rifle is beastly. Zaeed is obvious for any geth missions or often blue suns missions. I have always loved Garrus but he is not quite as insane as some others. Miranda is nice an versatile and Warp works very nicely with a Vanguard. Thane is an absolute staple on almost every missions for my vanguards. Sniper + Warp + huge dmg boost from his prsonal ability. Its nuts. The companions that I don't see working at all would be Jack since nothing she has is really going to help you. I don't like Tali much but she can strip shields and provide a drone to distact enemies while you charge and slay them. Its kinda too bad Tali has never been powerful since I enjoy her character so much. Legion I never use just because. Grunt actually can work great on a vanguard too. Send him up close as a distraction for you and then the both of you lay into people with claymores.

Planning on doing another Vanguard. Can't remember how I lvl'd him, so what would be best?

Passive > Charge > Bonus > Shockwave > Ammo
Charge > Passive > Bonus > ...

All joking aside, here is how most Vanguard players level up their Vanguards:
1) Max Charge to Heavy Charge
2) Max Passive to Champion or Destroyer (does not really matter)
3) Max Incendiary Ammo to Inferno Ammo

Your charge-bot is now complete.

From here, you have some options:
(A) Charge-heavy build:
4) Max Cryo Ammo to Squad Cryo Ammo
5) Max Slam/Shockwave/Reave or other bonus power. Most take Slam since it is quick and can set up Warp bombs.

(B) Charge-Caster hybrid
4) Level up Shockwave to rank 2 to unlock Pull
5) Put at one point in Pull.
6) Max Pull to Pull Field -OR- Max Shockwave to Improved Shockwave
7) Max Shockwave or Pull
Stasis would be a good bonus power to round out the build (requires Lair of the Shadow Broker DLC). You can always replace Pull with another bonus power such as Reave or Slam. I strongly advise to never invest or use Cryo Ammo if you want to cast Shockwave or Pull/Slam as a Vanguard since it can interfere with biotics.

I like Slam as the bonus. Fast cooldown, works well at Level 1, sets up warp bombs.

^ I can vouch for this build. It is surprisingly versatile and powerful.
