Ok, so I just completed a Gold match against Reapers at Goddess, and the weirdest thing happened. On wave 11 there was a connection issue with the host, so he dropped out of the game, and we restarted the wave. Everything was normal from then on, until extraction. The only other guy that remained in the match and I were both in the extraction zone when the countdown hit 0:00, and we got the "full extraction" achievment notice in the right hand corner, but then when we got to the score menu, it said that we had failed the mission. WTF?
Has this every happened to anybody else?

Always happens if the host leaves on wave 11. Doesn't affect your credit or EXP yield at all. Probably means your extraction won't count towards the Squad or Allied goal, though.

Yes, a few weeks ago. The host left on wave 10, restarted with only two, and we got a full extraction, but a mission failed. Well at least we got paid that time...

ive had this happen to me, however on on extraction. ive jumpped into game and team was doin objective on wave 3. we completed it and it said objective failed. but nothing really happend still got credits so i just ignored it.
another time everyone bled out and i was on last sec during extraction and i got downed. didnt make extraction but completed mission. ( think this one happens more often)

Yes, I got that once, same issue with the host leaving before Wave 11 starts.

Yes, not on a Gold though, and not on extraction, on a objective wave but we continued anyway.

Ok, I'm glad it wasn't just me going crazy. It's a bummer it happened during this weekend's operation, though.
