Picking up weapon mods glitch

I'm not sure if this is a problem with my save, game or patches, but sometimes when picking up weapon mods, the small popup window at the bottom right doesn't appear. If I go to the weapon bench and try to equip the mod after the mission, it doesn't even appear there. This is very annoying.

This is obviously a glitch. I have read on these forums and come to the conclusion that the more playthrough’s you do the more likely you are to encounter different glitches.
Which actually kills the replay value off this game.
It’s also quite clear to me that Bioware does not like the PS3 system hence the obvious lack of responses in this forum compared with Xbox and PC users.

I've encountered the same problem (pc version) - sometimes loading a previous save helps but not always. Looks like both priority:mars and priority:palaven are affected. It's quite annoying and prevents me from advancing futher with my me3 playthrough. Hope bioware guys have scheduled it for the nearest patch iteration...

I've encountered a similar problem. However, I noticed that it has only hindered me in obtaining the AR Extended Barrel, the Shotgun Spare Ammo and Blade Attachment, and the Pistol High-Caliber Barrel and Scope. Now on my third play-through, these are the only weapon mods that have remained at lvl 2 (I could only buy them); all others are at lvl 5. My Shepard scans the aforementioned mod with his omni-tool, but it doesn't seem to register (the pop up in the corner doesn't appear), nor will it be at the weapon's bench.
I find it odd that this particular bug (for me, at least) only affects these 5 mods. Even more so, however, when I watch my girlfriend play through the game and she picks up each mod with no problem whatsoever. It's maddening!
ALSO, I've tried saving/ reloading before picking up these particular mods with no success. Madness continues...

Okay, just performed another test. What was supposed to be my third run through the game on Insanity quickly became part of my experiment. Switched it to casual, breezed through the game as fast as I could, saved before cerberus Hq, and then I deleted the game data "DLC and Patches". Re-downloaded the necessary DLC to continue my game (From Ashes & Extended Cut, specifically), turned off my internet connection, started game WITHOUT installing the new patches, beat it, blah blah blah...
Started fourth run free from the patch (New Game +4), made my way to Sur'Kesh (a stage with all the mods that I couldn't pick up before because I BELIEVED it was the work of the patch), and to my displease, I STILL couldn't pick them up.
So, I don't know what's the deal. Maybe having the patch installed before affected the save data to where any following playthroughs prohibit me from getting these mods (basically meaning deleting the patch and dlc won't do diddly squat), maybe its the extended cut that's the cause, or some other factor...I don't know. Frankly, it's too much of a pain to bother with at this point. I'm not even playing the game for fun anymore. These past couple of playthroughs have all been for the sake of finding out what might be causing the problem.
I'll just put this game on the shelf for now so I don't tire myself out with these continuous test runs, and just wait for a new patch to come out within the next couple of weeks (me being optimistic, cuz I'm sure it will probably be a couple of months).
Will keep checking back here to see if somebody pinpoints the cause of the problem/ discovers a remedy.
