How do these skill tree issues keep happening?

I'll say this in advance: I realize that coding a game is hard work. I realize that I'm ignorant of how devs' jobs are done, and I don't hold much weight in this conversation. That being said...

I can't remember a game in which the listed in-game effects didn't actually happen when you chose a power-up, let alone how many times it's happened in ME3's multiplayer. Phoenix, Justicar, QFE, now the Shadow... Half the time when I spec a new class, I feel like I need to quickly search the forums to see if there's a glitched skill evolution I should know about so I can avoid wasting points on it.

Maybe I'm just ignorant of this problem in other games, but I don't remember ever in an RPG upgrading a skill only to find it never actually upgraded. Even a game that's been widely recognized as a rushed, glitchy mess-- Resident Evil: Operation Raccoon City-- doesn't have this problem in its class-based skill system. When I purchase the lvl.3 skill of a character, it works as advertised. No problems.

So I guess my issue is... How does this keep happening in the highly funded finale to, arguably, Bioware's key franchise of this generation? How is it that even when they say in a patch's notes that, for example, the SMG ULM bug has been fixed, when the patch actually comes out it's still not fixed? How long do they test this stuff before they release it?

I'm still enjoying the game. It's just bugging me. There are far cleaner games out there to play, and I feel like ME3 should be held to a higher standard.
