SS's definitely got some "not your fault but your dead anyway" bugs.
It helps to make sure they lose you as a target before using SS. Might take a few seconds after you cloak.
It won't matter if the enemy detected you or not before you SS. It's that split second when you triggered the SS, the system has already placed you behind the target, ready to strike, not yet struck, and the enemy detects you and starts turning. If you appeared right behind, I don't think any enemy has the turn reaction time to fully get you into their front arc. But if you appeared at the side, that reaction time and turn speed can get you into their front arc -- and firing line.
The most reliable way I have seen to work, is that the target is already preoccupied with your teammate. That way, he will point to your teammate, you will appear besides him, but because he is concentrating on your teammate, they will (likely) detect you but not turn to you. Then your SS will hit for full. This is most evident in Guardians, which as noted, should by rights be dead on every SS... but doesn't.

In laggy games, I do notice that my Shadow immediately appears behind the target, then POV swoops in and I slash. Almost seems like the body teleported and the soul has to fly back into the body. Before I maxxed cloak, I would see my uncloaked self standing behind or beside the target (as activating SS breaks TC, and the cloak part of SS apparently doesn't happen until your soul catches up to your body). With Bonus Power evolution, I see my cloaked self standing behind the target as I swoop in, since TC hasn't been broken yet. Still need to be careful against boss enemies, as they seem to be able to see through cloak pretty often.
Still sometimes whiff on Guardians if they have there isn't room to spawn behind them, but at least I'm still cloaked and can either run for it or just shoot them with a piercing mod on my gun.
Try it with duration and bonus power and see how it works for you. With both you can even TC--SS, wait for recharge, SS again within one cloak. Just remember that the second SS does break the TC immediately. You might also have better luck as host.

huh? I SS Pyros all the time - OHKing them - on Gold (after the Martial Artist proc and letting the electrical damage over time finish the job.) If they're alone I can finish them off immediately with a heavy melee.

It is - it only applies to your light and heavy melee, but not shadow strike. Still, it's only 50 damage off of a massive amount of burst damage so doesn't make that much of a difference.

Edit: I also love using ss on phantoms without their barriers. It's risky, but fun.

Best way to make sure you don't get hit while shadow striking is to wait until your target is focused on someone else. Cloaking and straight away shadow strike is not enough, wait for a second until your target turns the other way, then get him.
I like to cloak and then do a cartwheel to the side first. Enemies will still shoot at where they saw you last, so that avoids getting shot, and it gives the enemy time to pick a new target, at that point I can safely shadow strike.
