In the Final Battle quest, I took my group to the alienage from city gate and killed the darkspawn general (with a quest update acknowledgement) and then took my group to the market district and killed the second darkspawn general (again with a quest update acknowledgement) the headed my group to the palace.  Travel was interrupted with a battle back at the gates with the rest of my group.  Before anything happened I pause the game and open up my journal to find that it has changed, and now it reports that the alienage was overrun and the darkspawn general was not killed.  What's going on?

If I continue, I get to Fort Draken but the door going up from the second floor is not "active" and I can't continue on.  Can anybody give me any ideas what's going on?

Odd but there are 3 generals perhaps you missed one.

I feel like a dumba**, found the right door on the 4th try. The ending was great!
