Silent Hill - It was my first survival horror game. I was pretty much freaked out when those demonic baby things started to attack Harry. Also, every time I visit a hospital in real life, I can't stop thinking of Silent Hill.

Imagine having worked for about half a decade on the first game of a series, and said game is ultimately both critically and (perhaps more importantly) commercially successful - a product anyone could and should be proud of - as well as one of the rare few games in its genre; anyone wanting a similar experience would be forced to come back to this very same developer.
At that point, the logical step as to creating a sequel would be to follow what is nowadays most commonly known as the Call of Duty formula; keep the same core game, change the setting, write a new story, add a bunch of new attacks, spells and equipment, and polish/tweak some things which need that. Et voila, a perfectly fine new game which would sell about just as well as its predecessor. What more could anyone want?
Instead, after the success of the previous game, they instead chose to overhaul everything to the point that the resulting product in no single way resembles that which came before it, and the lack of logic behind that decision just right-out disturbs me.

Gameplay that only revolves around beating people with baseball bats until their head pops open and the brain comes out in pieces or stomping pixel men to death while they're rolling on the floor begging for mercy is kind of disturbing, even by my standards.
I have no problem with very graphic gore in games but making a games only goal to kill people as brutally as possible and glorifying it like that is kind of tasteless.

When I finished that game I felt like I needed 50 showers. Creepy baby-things coming at you and that child? Good lord, get me out of here!! It didn't help I couldn't save that woman at the time and always got the worst ending.
Also, Alan Wake. I'm not even sure anymore about what's real and what's not about the story. I give to the game's writers an A++.

Dragon Age II, although not necessarily for anything in the game but for the game itself as a product; as the logic driven person as I am, I cannot for the life of me comprehend several decisions that were made in the development of this game.

Bro, you've only scratched the surface of what makes it disturbing. You left out the part where it gets glorified by a creepy fanbase for it's sub-fan-fiction level "romances".

Honorable Mention: End of Dead Space, the last scene freaked me out. It's a shame EA's doing it's best to trash the franchise now

What a great moment that was. Radovid is just something.

I have a bit of a resistence built up to gore, but psychological creepiness is what gets to me. Slender is really freaking simple but it's the scariest thing I have ever seen in my life. It's like 15 straight minutes of the one rule we as human beings can never obey: never look behind you.

The most disturbing game I played I don't even remember the name of - it was a Japanese anime type game I found which looked innocent enough (hey I was young and naive), so I gave it a whirl - it started out nice enough but then after a while it started to get pretty... twisted
Also, Fallout 3 has to get an honourable mention for the most disturbing mods ever, so shudder-inducing that they put the twilight and even bioware fanfic communities to shame with their shameless depravity and mind-boggling sexual deviants.

Does real life count?
That game is too hard, stuck on level 19 at the moment, need to rank up my driving skills.
So far, currently doing 'Complete Engineering degree'.
I succeeded in leveling up a lot, but unfortunately so does the game! And balance is always way off, not really fair. The nerfs and buffs alone could drive a person crazy!
Yeah i know man, i'm level 19 but there is this boss called 'sister' which keeps leveling up with me, think it's level 16 now.
Might reverse pick-pocket her and slip in a grenade, but then i lose karma points and i get a 4 star police rating. Got no weapons on me either, just need to find a weapons trader somewhere. Anyone fancy doing some trading? I'd trade my shirt, it's blue but no armour rating or upgrades.
Oh, and the bar fight scene with the boss sister will be great in the future. It uses the exact audio from ME3, "Time to reload, enemy incoming!". Of course, reload is in the bar context. Plus, the microtransaction system is all over the place, you really can't get ahead without buying points.
BTW, have you guys seen the endgame on it? It SUCKS.
ps: re: engineering degree - I found that farming beer and casual encounters with the opposite sex a lot helped levelling through uni go faster, but YMMV if you don't like grinding.

You can hear breathing, you edge forward and the camera view quickly changes to behind you, and in front of you, you find the source of the breathing. A mutated replica of Eileen's face staring at you, her eyes just following where you went. Didn't get a 'what the...hell' from Henry either! It's that disturbing!
There was no surprise creepy sound played when you see her face, it just watches you...

Also, escorting Eileen had to have been one of the worst escort segments i have played in gaming at the time. It was more tedious than guiding an injured Eva in Mgs3. In order to heal her you needed purifying candles which were limited, and they were needed to exorcise your apartment of ghosts.

Happy Wheels, Metal Gear Solid 2 or Saints Row: The Third. The Zimos moment was disturbing and funny at the same time. I think the next Saints Row needs to tone down the ridiculousness though.
