I got sick of the lack of cool Dragonbone helmets in game beyond Corruption, so I made myself a new one without issue. When it was exported as a DLC it appears in the DLC menu represented by a skull wearing a wooden helmet with no description beyond the title.

I'm about to make an upgraded version of the Silverite Crow Dagger for my next playthrough. Is it possible to assign a specific picture/icon to it for use in the DLC menu?

Yes, you assign it through the Module Properties box.
