Honestly though, are you referring to the three Levis with Shepard's mech in the foreground or something else?
- Shepard is on hands and knees, child walks up to him and says "Wake up."
- Shepard is on hands and knees, Ann walks up to him and says "Breathe."
The "Breathe." line is weird, it's clearly just there to mirror the "Wake up."
The similarities between the two scenes are meant to clarify that what's actually going on in the decision chamber. Because in Leviathan, you know you are looking at an illusion, because even though you see Ann Bryson, you still hear the Leviathan's voice. Plus, the first thing he says after "Breathe." is "Your mind belongs to me now." Then he goes on to explain that he's using your memories to build the illusion. ("Your memories will give voice to our words.")
Moments later, he explains that all Reapers have this power to influence organics, only the Reapers have perfected it to indoctrination. In other words, Reapers have a mind control power just like the Leviathans, which lets them build illusions from your memories, and they can do it even better than the Leviathans themselves.
The implications of this are driven home by the similiarities between the first moment of the Leviathan mind control and the first moment in the decision chamber.
Every time we see Shepard on hands and knees, there is some form of mind control going on.
- The blast from Object Rho gets Shepard on hands and knees.
- Leviathan's mind control gets Shepard on hands and knees.
- Shepard falls on hands and knees moments after Harbinger's blast.
- Beginning of the decision chamber scene.
