What was so wrong with the prologue? The only thing I can remember besides the phantom kid, which doesn't count for obvious reasons, is that there was a dreadnought in Earth's atmosphere.
Lack of explaination about what was going on. Especially for the new players the game was supposedly catered for, this would be very confusing indeed. Contrast to ME1 where the first section of the game is on board the Normandy, and you can talk to various crewmembers to get a bit of backstory.
Making the Alliance look completely incompetant. Suprise attack or no, they had time to summon Shepard to a meeting and then all they can do is ask what they should do. Admirals asking a Lieutenant Commander what to do. Yeah. You fill me with confidence, guys. Probably a good job the Reapers blasted them directly afterwards.
No proper introduction to Vega. Minor point, but supposedly he's known Shepard for six months and yet he's a complete stranger to the player.
No trial for Shepard. Loads of us were looking forward to seeing the Alliance's reaction to Shepard's previous actions. But we got nothing of the sort. Disappointing.
Otherwise, the rest of the intro felt like a tutorial. The 'tutorial' in ME1 was basically throwing you in the deep end, and ME2's tutorial was less structured and had a pace that allowed you to read all the log entires and even have a conversation with Jacob (if you enjoy such things...). ME3's was... Well, I didn't get much out of it.
