Haha imagine if someone doesnt know about this and he gets that ending then, he must be like wtf is that ****
I'm sure instead of fixing it they make an extended cut for that ending with more clarity about this mysterious nameplate
not sure how this can happen as someone at bioware should have checked every possible ending before releasing it, that guy/these guys did a pretty bad job

Well it's a glitch, but you didn't mod anything, right? This is 100% the real thing. That's why it's so amazing! Mmmmh, really you should burn this somewhere, next patch might fix this, and i want this to exist everafter - you know, before they put easter egg StarBrat carrying the nameplate in there.
Also, someone should make a vid with both Ash and Kaidan alive too.

Your'e right, unless you have an EMS between 1750 and 2050, you won't get this glitch. I did an everyone dies playthrough based on my lowest possible EMS list, (which you can watch here) with an EMS of 872, and lo and behold, there is no plaque memorial. If you vaporize, there is no glitch. However there is one funny thing I noticed, if you watch carefully, Joker actually elbows thin air, as he's supposed to shake off someone at that time, which person doesn't exist since everybody's dead dave.
