I just had a match in which half of the biotics I used had zero effect on the enemy. Most of my maxed out lash either went right through the enemy or disappeared on impact causing no effect.... not even a stumble. My maxed out slash is always hit and miss and I think it's due to any kind of incline the enemy might be on, no matter how slight.
Yet again, I spent most of the match having to hit every button I used more than once just to get the desired effect. I have to button mash just about every time I wasnt to get cover or dodge which leads to the majority of my deaths. Yet when I sprint for an escape I become a damn magnet to every surface imaginable. Simply put, the controls in this game are so bady tuned... and yes, I have tried other controllers which are in good shape. This control issue has been annoying me since day one and I can't believe I'm now just mentioning it.
The icing on the cake for my match was that our second retrievable object on wave 10 fell through the floor causing a mission failure at the end of the time limit.
Anyway, just another handful of reasons why I'm so close to shelving this game for good. EA can forget it when their new DLC comes out. This game is just way too broken to spend any money on.
Stuck retrievable object, biotics not working and unresponsive buttons.

Yeah, this tends to happen on PC version as well. It may partly be due to lag, although it seems to happen even when I host.
And as for having to button mash repeatedly to get certain things to work, that happens on PC as well (and having just got the Xbox version yesterday, I died several times trying to revive a downed teammate because the stupid revive just wouldn't register).
