All of the right settings are on and i completed the requirements more then once.

None of us are going to be able to help, unfortunately. Have you tried do the Live Chat with EA customer service? I don't know exactly how to initiate it, but I've heard of others with your problem doing so and getting their Commendation pack pretty quickly.
