How do you kill the four phantoms and kia leng himself on insanity

Focus on the Phantoms immediately. You know they will come, you know where they will come. Get in a good position when that happens, make sure everyone is up and healthy, have a reloaded weapon and then start taking down a Phantom's barrier, then execute when you reach the healthbar (this is where squadmates or your own powers shine because they can be CC'd).

For me, I used Area Energy Drain (Chain Overload works) to take two phantoms at the same time, then I used radius Singularity to trap both while I shot them to death. Then repeat and after they are dead, focus on Kai Leng.

It would be helpful if you mentioned build, class and squadmates (and their builds?)

Here is how I did it. Skip to 4:17 to start at the Kai Leng fight.

I used the Claymore shotgun only. Liara was my second squadmate. I believe EDI and Liara are both using Scorpion pistols. 
I use the squad version of cryo ammo. 

You see me use Liara' Stasis a lot. Kai Leng is immune to Stasis but he also seems to avoid it so I put it up and try to keep it between him and me. I also Stasis the Phantoms since they are not immune.

I cloak and then go hunting. I have the "Fire One Power evolution of cloak so I use that to shoot Incinerate a lot. 

For another video, here is Trinity82x showing how to do it. She uses the Black Widow and Talon. Defense Drone is her bonus power.

oh sorry I have fitness up to max more weopon damage I have tactical cloak max with damage and cyro ammo max out im an infiltrator and I have of course EDI and garrus EDI has decoy at max defeance matrix at max and incinerate half way then garrus has pproimity min maxed and overload maxed and a few points in both in concussive and in armour peircing

Put up Decoy, have Garrus Overload a Phantom, then EDI incinerate the Phantom while they both kill her. In the meantime, you kill a Phantom on your own with Tactical Cloak and what not. 2 shots to the head and one will die.

When they are all dead, cloak, revive the squadmates, and murder Kai Leng.

but their like ninjas as soon as you shoot her in the head like you say she just starts fliping and floping all over the damn arena then she hits me with that hand blaster and in like three shoots im dead it's freakin fruterating or when I focus on one phantom Kia leng ends up using some sort of kamehameha blast of doom with his sword that almost kills me to I cant handle two ninjas at a time

I recommend using both EDI's and Garrus's Overload on the Phantom's barriers, followed quickly by your own Incinerate to tech burst them (one Phantom at a time; do not try to get them both at once). Not sure what weapons you have, but cloak and shoot also works; better when their barriers are down.

Infiltrator with Mantis, Katana and Predator.

I had Energy Drain as my bonus power (best one to take as an Infiltrator really), although I fired the Mantis with Disruptor then used Incinerate for Tech Bursts most of the time.

Honestly the above was somewhat sloppy since it was after I had focused on MP for a while and my reload canceling was off.  In any even the first time I played it with Infiltrator I had the Black Widow and you can make short work of them with that.  With the Mantis, just make sure you aim well and make effective use of your squad's powers, as Red has already said.

As capn233 points out, you can tech burst with Disruptor Ammo. You have the Carnifex, so you can slap Disruptor Ammo on it and go one (or more) of:
1) Shoot Carnifex > Your Incinerate (tech burst)
2) Shoot Carnifex > squadmate Overload (tech burst)

Also, if you put points into Sabotage, you can: Sabotage > wait for backfire (enemy will light up with electrical sparks after 1.5-2 seconds, barrier/shield should decrease) > then hit it with Incinerate/Overload. If the enemy is still alive, shoot it.

Just get Energy Drain and spam it OP. Also get squadmates with chain overload and use that often.

That fight isn't really hard on insanity, just annoying.
