Fairly simple really.
Get all the 2-h talents
Make sure you have enough HP so that you don't die incredibly quick.
STRENGTH!

Stack strength. Get the chasind great maul as soon as possible. (Two treaties + urn.) I prefer dwarf for the innate magic resist and the fact that they are simply amazing, but race isn't too important. Other than that, get all the fade bonuses and get sunder arms/armor asap as they will eventually be your bread and butter moves. (End game doing 200 damage a shot.) You will need a mage for haste. It's my favorite spec by far and does perfectly fine on nightmare difficulties.
Specs (This is personal.) My favorite is templar for the innate magic resist and CC cleanse. As well as a ranged aoe knockdown. Then champion for superiority. Others go reaver/berserker etc...

Thanks guys, real helpful, I had no idea about the Sunder attacks... Does anyone know if mightly blow increases your odds of getting a "shatter" effect on frozen/petrified enemies? in my experiences it does about twice as well as a standard attack. (and it works on elites, even at full health)

Mighty blow's an auto crit, so it should always shatter. Yellows will only shatter if you're on an early patch though

Noticed, on my last playthrough, that the chasind maul is available also with three treaties, without the urn. So I guess it's just three major missions.
