Shepard does somehow survive this ending according to BioWare.
Surviving the explosion is not that big a deal for me. He was clearly on a shielded area of the Citadel (since he was not dying from being exposed to the vacuum of space with no helmet or armor). Who knows how much of that explosion those shields absorbed. Since he breathes at the end, that shielding must still be in tact (once again, he couldn't breathe if exposed to the vacuum of space) even after the explosion, which would imply this area is protected from the blast. The rubble he lies in is apparently the rubble of the power conduit that he blasts to crap. Jessica Merizan makes it clear that he is on the Citadel and did not fall to Earth on her twitter feed. When asked how Shepard could survive re-entry without a helmet in the extended cut, she responsed "@TheDaniCita re-entry? As for his synthetic implants, we are not sure how much of him is still synthetic after he has been healing for quite some time since the Lazarus Project. For all we know, his vital organs may be able to survive without the synthetics by now (apparently they can based on the breath scene). I'm sure he is in really rough shape, but if they rebuilt him from being brain dead once before, I am sure they can fix up a dude who is still breathing on his own if they find him soon enough (even with damaged technology from the Crucible firing).
