Insanity infiltrator help

Hi there i am going to start my insanity run soon with my infiltrator and i am looking for a very good decent build to use.
Also can anyone recommend me an amazing bonus power and armor pieces and an ideal squad to use.

depending on your weapons preferences... here is what I used.

Infiltrator with Energy Drain bonus power. increased everything except cryo ammo (ignored it for the additional damage of disruptor ammo)

By the mid point or so, a Valiant V with EB+concentration, plus my fully damage % armor components, and intel, was able to one-shot troops from cloak (damage bonus to sniper rifles). Just cloak, ED (if shielded), shoot.

I ignored fitness for the most part.

For squadmates, it depends what kind of opponent. For organics (reapers) I used liara/javik for biotic combos. For Cerberus I used Garrus/James. For max grenades I used James/Javik (and my own sticky grenades)

On my first Infiltrator playthrough, I ran something like this:
6 - Disruptor Ammo: Squad, Headshot, Stun Chance
6 - Cryo Ammo: Squad Bonus, Headshot, Freeze Chance
6 - Incinerate: Radius, Burning Damage, Armor Damage
6 - Tactical Cloak: Damage, Recharge, One Free Power
6 - Sabotage: Damage, Recharge, Tech Vulnerability
0 - Sticky Grenades, point sink
6 - Passive: Damage, Weight capacity, damage
6 - Fitness, Health, Shield Recharge, Health
0 - Bonus power, point sink

I am playing it again, and I would change:
Disruptor Ammo to Damage, Headshot, Damage
Incinerate rank 4 from Radius to Damage

As a bonus power, you could run Decoy, Energy Drain (probably the best bonus power), Proximity Mine, or Defense Drone (sniping playstyle only).

I have played infiltrator from a brand new character and an import on insanity... here are my thoughts.

Cloak is of course the most important power and one of the best in the game. Unless you are not using Sniper Rifles, you should go max damage. The sniper rifle bonus for Cloak is an odd multiplicative bonus. More on that later. If you absolutely do not want to use SR's, then you could potentially consider Bonus Power at Rank 6.

Disruptor ammo is nice. Try to max it and go max damage. You can take the squad evolution if you want to try to get more Tech Bursts, but I do not think that is necessary. Mainly you want at least some DA bonus so that if a target starts to regen shields you don't waste a sniper shot.

Next thing to consider is what rifle you want to use. You can get through the entire game with the Mantis if you want, despite the lamentations about shield gate. There are a couple ways to go about this. With Disruptor Ammo, an interesting way is to shoot them, then cast a power under cloak to set off the Tech Burst. That gives you more CC and early game usually will kill basics (late game it doesn't work as well, at least not with level V weapons). Alternatively, you can hit them first with Energy Drain, or Sabotage with Backfire, then shoot.

If you are planning to try to get the Black Widow ASAP, I feel the bonus power is actually less important. Reason being is that you can basically kill everything in the game just by cloaking and using the Black Widow. The same thing somewhat applies to Valiant, although it is not quite as good on infiltrator (time out of cover, cloak rhythm...). Your shots with Black Widow can all get cloak bonus as long as you fire them as close as possible. That means if you also want to use a power, it should come after the first shot. Of course, if you really want to fire the power first, you can... but you basically can't get three cloaked bonus shots on the BW that way.

Operational Mastery needs to be for damage, weight cap, and damage as Red said. Also the last SR damage bonus in the passive is actually a modifier for the multiplicative damage bonus in Rank 6 of Cloak... meaning if you take both your total sniper rifle damage (after additive bonuses) is multiplied by the total bonus. It makes cloaked SR shots very damaging (and why Black Widow is again a better choice than Valiant).

Max fitness when you can. With Cloak you can avoid some damage.

Sabotage should be Backfire, Explosive Hack, Tech Vulnerability. Use as your opening power on large targets.

Incinerate is decent to have, max damage / armor. Use it as the second power against armored targets that are still left after the Sabotage cloak cycle.

Sticky Grenades are not all that useful, unless you want to go max tech bursts.

Bonus... depends. You don't actually need one, but of the available choices Energy Drain or Proximity Mine are probably the best. You can stack debuffs from your Sabotage or Proxi Mine with Garrus's Proxi and Tali's Sabotage...

Oh and a bit about Armor.  Just go Hahne-Kedar if you want the "best" for an infiltrator.  You don't really need more headshot damage.  I typically just go whatever I think looks cool and a lot of the time that is mainly N7.
