
Step two: While in spirit form, turn into mouse form or cancel spirit form. If you were standing in the right spot, you'll appear above the ground, on a ledge. Walk away from the lake and the actor is re-located underneath the map:

Step three: Wander around in the fade:

Step four (how to get out): Find areas off the map which have large roots, you can walk up them if you're careful. Done correctly, you can climb up the areas outside the normal walk-zone, and even onto the tendril-like formations. Once there, you can return to the normal walk-zone by walking through the edge, or falling down from a tendril hanging over the walk-zone.

In other words, the bug is a vertical placement calculation when changing forms that's overdoing it. It likely works in other fade areas too.
