If everyone survives the Suicide Mission in my first play through, then some people die in my second, which carries over to ME3? Thanks in advance.

Related question: I've tried about three diferent characters now and no matter what I do I loose two, or more of my party. Is there a "magic" party that makes it thru the final area intact?
1st character) I sent Tali into the air ducts, lost Jack at the "bugs" door, then Thane got shot during the hold the line behind the seenes fight. Then on the way out of the area Samarra was crushed by falling junk. She was crushed by the steel girder that landed on her.
2nd character) I lost both Garus and Tali, I don't recall where or how.
3rd character) today: Legion was lost at the bug door, and Jack was lost during the hold the line fight.

Typically i myself will always use Tali or Legion for the ducts as they are the obvious tech specialists.
But i have been successful using Kasumi as she is good with tech, and an expert infiltrator.
Garrus is always my first choice to lead team 2, but have been successful with Miranda.
I always ensure my team is loyal, and i have every frakin' upgrade in the game...cause im a nerd like that...

After I posted that I found the walkthru post.
The entire team but Jack is loyal, I told her to ****** off during the cut sene after her mission. Ysing the walkthru notes I was able to find the magic party Now I can start ME3 with all of the characters in the background for the first time.
