I completed the requirements as well and I get the same "did not complete" status as well. I put in a review but have yet to hear anything. I think at this point I'll have to contact chat support and ask for my pack.

If I don't get it, no big deal.

EDIT:  I contacted EA chat support and was immediately granted the commodation pack.  Say what you will about EA as a company, but its support staff is very pleasant to deal with.

Hah I didn't even realize there was a challenge going on... Was happy to see both packs waiting for me. Was not happy that there was a Hurricane II instead of a Valiant. :'(

I used the Commendation Pack Help page but after 10 hours nothing happened. I contacted EA help vie email and they gave me a Commendation Pack right away.

Shouldn't we get both a Commendation Pack and Victory Pack? Why did I only receive Commendation?

Normally (as in all previous cases) I had to contact EA support to get the commendation pack. The victory pack would show up just fine. This time, again, no commendation pack, but I clicked on the link at the top of the page of MP discussion home page and "appealed" my not being awarded a commendation pack (i.e. clicked the link). That was yesterday, today I was awarded the commendation pack. The first time I have not had to go to tech support. Not perfect, but they have a much simpler workaround for folks like me, who earned a pack, but didn't get it in the pack distribution. Eventually they will fix whatever glitch is causing this, but for now, thanks Bioware for make it easier to get the pack. OBTW, got a Crusader IV. I'm not normally a heavy shotty person, but may have to try it again, now that it has received a few upgrades. Now if I could just upgrade my Saber ...

It says no for me, but I did a bronze london run (slumming) on Saturday with full extraction. Review clicked. Let's give them a day or two, then phone call time (again). This is the 2nd time this has happened.

The new solution worked for me as well. Took a little less than 24 hours -- but then again, I usually try EA online help by Thursday morning, so it's a net gain.

I had a slight issue: after purchasing the Commendation pack I received Targeting VI II (I have been trying to upgrade this forever now) except when I went to equip it to a character my manifest showed that I only owned Level 1. I remember distinctly that I received it because i noticed it read "Headshot Damage: +11%".
At any rate to who do I report that I experienced this error?

Yup, I'm from Singapore too and face the same problem every operation. None of the Singaporean players I know have ever gotten their packs normally either.

I have the same problem (I've never gotten a commendation pack without contacting support, this is my third success) and am in the USA. My copy isn't imported either--it was a preorder from Amazon (the US site).

Also from Asia, and according to the help I have no com pack, and I'm certain I did full extraction from FB London several times. I have always gotten my com pack by contacting live chat. Hope we don't have to start recording our extractions as proof.
