The concept art is interesting, and the story and character summaries are all believable.
But wasn't Bioware tweeting about VO work the other day. If they're already recording lines, isn't it a little late to be asking people what they think about "possible" companions? It seems they'd be past the possibility stage with characters if they're already recording. I bring this up in case anyone knows more about how much fluidity there is with the writing of characters when VO has started.
Keep in mind this seems like a marketing survey more than anything, aimed at a very mainstream audience. So its likely being used to figure out what non BioWare, non RPG fans might be looking for in the marketing of the game.
So it'll determine how many times we hear David Silverman say "Awesome!" and "Visceral!" and "Epic!" over and over again because their market research (like this survey) told them that people liked those words. Or maybe how much marketing chooses to focus on a given companion in the marketing because a bunch of people thought the concept art or description was highly rated (or conversely, a companion who isn't rated highly ends up not factoring into the marketing).
As I recall, back in Fall of 08 when Origins was still slated for a Spring 09 release, they had a video where they introduced Morrigan and it was a different voice actress and different face model than the final game. So I imagine that even if they're recording voices, they could still change things up visually for the characters, but the writing of the characters is likely set in stone.

Could just be a publicity stunt. I guess
we'd need to know exactly when the survey was posted, in order to get a
better idea.
Its legit. The concept art seals it. Unless somebody managed to perfectly emulate Matt Rhodes' artistic style, the survey is totally legit.
That's not to say some of these details might not change, but the survey is so very clearly officially from BioWare/EA.
