Mine was this from Super Smash Bros for N64.
www.youtube.com/watch
Super Smash Bros has a really creepy main menu music, and then I get into a really scary continue screen when I lose all of my life stock, and then I press "No" in the continue screen, the game over screen appears with a very scary voices, and a scary musics that started. That's how it scared the life out of me as a kid, including during thunderstorm where it got me into a fear and almost got panicked because I thought the lights in the house is going out.

Scariest game over screen? I think this qualifies. This scares the **** out of me and I haven't even played this game.
In case you're wondering what that ripping sound is, that's the sound of flesh being torn apart.
Wow.
Why did I click on this thread?

Alone in the Dark: New a nightmare. Watching my older cousin give a mirror to a ghost in a mirror I think and she turned into a skeleton. That gave me nightmares.
Lol, I remember that - that's actually the only thing I remember from that game. I instantly had to go Youtube it.
As a kid, I found the pre-game over music from Uninvited to be a three second loop of pure terror. (if you heard this music, you were dead, so it's practically game over music).
Edit: here's one of the more well-known deaths from Uninvited (plus all the rest, should you care to watch): Clicky

For a terrible game, it had some really awesome death scenes. I wouldn't recommend playing it, but you can see some of the death scenes in some "Let's Play" scattered across YouTube.

There's this Game Over screen for a Home Alone game on the NES, it shows a red tinted screen shot of Maculay Caulkin (I'm not going to bother to spell his name right) looking like a demented sick kid. I think I read about it on Cracked or something.
OH NO!

Sheeesh, that would give kids a nightmare while playing that game. You always get freaked out while playing "Isle of the dead" while you get a death scene and a game over? By the time I click that link, and you want me to skip to 3:30, I saw some cruesome death scenes and then get to a laughing scientists game over screen. I mean, that was freaky.SOLID_EVEREST wrote...

Here's one of the tamer bad endings.
Of course, if you don't play Corpse Party at night and with earphones, you're doing it wrong. Although the shrieky teen girl voice acting (there's a lot of it) may destroy your ears...

Yeah it's not scary, more like awesome. It's my favorite of the bad endings.Shepenwepet wrote...

...while not "scary", at least for me, this ending to 999 stuck with me.

Ellie. (After DS, I was just totally expecting them to throw that out there again.)
That counts as game over. Right? (The many deaths of Isaac Clarke would be the other game overs but they just didn't give me the creeps as when he looked to where Ellie was supposed to be sitting before we see what's there.)

This reminds me of The Dark Knight.
I nominate Banjo Kazooie.

Why would I resurrect this thread? Because I have other scary game over screens that I was going to talk about.
When I was around 16 or 17, I buy "Blazblue: Calamity Trigger" at Best Buy. And what got to me when I play this game is that I lost the battle in the story mode where I face against Hakumen while I play as Ragna, and I got this game over screen that really got me jumped and I was like "Only one chance, then it's over?"
www.youtube.com/watch
Also, when I was a kid, I went to "Golf N' Stuff" on a field trip, and I saw somebody played the arcade called "Carnevil", a arcade shooting game, and then they got a game over that I saw that really got me feared because of the creepy image of the final boss, Ludwig von tokkentaker, and the writings that said "GAME OVER" were pretty big as it is.
Also, I thought the game over screen from Yoshi's Island were pretty scary to me when I was a kid.

(might want to turn off the annoying annotations)
That whole game was bizarre and kind of creepy for a little kid though.
