My game froze just as I told Duncan I was ready to leave Denerim... and after I turned it back on and re-did the entire City Elf origins story, nothing unlocks

The game must think I already have the achievement, but if you check my gamertag "Kaldo" (http://live.xbox.com/member/Kaldo) you'll see it's not there.

What gives Bioware? Is this something that'll be fixed in an upcoming patch?... or will I forever be stuck on 1090 gamescore, once I finish the rest of the game?

I'm a completionist... so if I can't get my score to 100%, i certainly won't be bothered buying and further DLC that comes out.

Please let me know if any solution to this problem is either available or on it's way... Thanks.

completionist. Never heard that before. If isn't really a word it should be; it applies to almost everyone with ocd.

you may have to start the origin again. That's all I can think of at this time.

Achievements not unlocking is common in this game. And broken achievements will off course be fixed. Its just a matter of time.

If this was a Bioware only product, I wouldn't doubt that Master Bandit... but since EA is now a part of this whole thing, i'm not so sure... the amount of EA affiliated games with buggy achievements that are never getting fixed grows with every release

i ran into an achievement bug too a few mins ago. seems your not the only one. my one didnt pop up ingame, but is claimed here on this site. it was a quest for the mage community, a quest from that mage behind wades shop. dont know the english name, but I have posted the bug into the german support forum already. German EA support link is down for me since a few days...coughs.
