There is no reason why people in Orlais (France) would speak English with a French accent amongst themselves.

This is partly true. The Free Marches were inspired by the old Germanic/Italian/Holy Roman principalities and city states. Genoa, Venice, and the Hanse are examples.

I am from one of those places (a hanseatic city-state), and Kirkwall could have fooled me.

Seriously, they are just a bunch of ill-defined generic fantasy city-states of the kind you can find in a gazillion fantasy settings. In fact, I barely recognised Kirkwall as a city, let alone a state. Lastly, two words: 'Sword Coast'.

I've heard Stephane Cornicard speak English almost without an accent. It's a bit more pronounced here, almost certainly deliberately. And probably kind of ironic, as I seem to remember reading that Napoleon spoke French with a noticeable Italian accent all his life.
As for Marjolaine and Isolde, yeah, that was an American voice actress, Kath Soucie, a born New Yorker...
I think that what we hear in DA:O and DA2 are non-Francophone actors mimicking an exaggerated French accent, and French actors doing the same. Call it Pepe le Pew syndrome (though Pepe's not so bad, I guess, and based on Maurice Chevalier's trademark film accent).
Or maybe 'Allo 'allo syndrome...
