From a purely business point of view the end has to be modified by BioWare one way or another. Be it by 'project X', the last piece of dlc, or by the beginning of Mass Effect 4.
The ending of ME 3 is just too messy for BioWare to continue a successful franchise with a sequel. In my opinion there are three ways to continue the current story/ending of ME 3 in ME 4.
1. They canonize or retcon the choice you had at the end and do as if the original endings never happened or mattered much. Since so many people felt the ending felt so rushed and (at least for synthesis) came out of the blue and felt as if nothing you did mattered, this seems to be a bad way to get your fans to buy ME 4. Another way of doing this is by setting ME 4 in the far future and by doing this making the end of ME 3 less relevant. This, in my opinion would be the easiest way out and would aggravate me as an ME fan the most. It would also be the final nail in the coffin for my ME experience and personal proof that BioWare really doesn't care about consistent story telling within the ME universe anymore.
2. They release 'Project X' or the final DLC, which either somewhat changes what we currently have, or takes place after the breath scene/current endings. And by doing so can both show us our war assets / show us our made choice, and still work towards a valid starting point for ME 4.
3. They write option 2 as the opening sequence of ME 4.
Again, from a business point of view, it really doesn't make sense to skip a whole story part and pretend nothing happened when starting ME 4 and expect your fans to still have faith in your capabilities to deliver a great story and emotionally engaging franchise. The new work from Sam Hulick, all the writers being on board, and many squad member voice actors confirming they've been back in the studio shows us something interesting is going to happen. I just have this last piece of hope this will fix most of the problems we have with the ending. And if not and this last piece of 'Project X/Citadel DLC/Ending' dlc or whatever you wanna call it turns out to be another 'detached-from-the-main-story-side-quest' then I'm done with BioWare and Mass Effect, and I'm not saying that as an angry customer, but will be saying that as a formerly devoted but ultimately very disappointed Mass Effect fan.
