Hmmm, he almost never dies for me. What are your tactics for him?

Perhaps you should stop using him as tank then?

Does he still have the same crap equipment you recruited him with?

Want a suggestion? Tank should head in first always.. Remove AI-tactics.. Make sure your tank taunts all enemies.. If he gets agro stun and run away, or fight if you see fit. A rogue should never die.. If one does, the fault is yours as a player I'm afraid..

Micromange him.
I've found the auto-tactics for him are godamn awful. He'll try to tank any yellow or above mob by himself because of the Mark for Death one. He'll also use Dual-Weapon sweep with impunity. And if you never micromange him, he'll always be face-stabbing rather than back-stabbing.
Right now i'm using him as part of my Warrior char's playthrough (Nightmare), and if he dies, it's only because I got stunned and a yellow mob decided to use him as a chewtoy while Stealth was on cooldown. The tactics I have for him are as follows: Self - Being Attacked by Melee Attack - Stealth, Self - Any - Momentum. (I've also got Song of Valour, as i'm using both him and Leliana) That's it. If he does gain aggro he'll likely ditch it by using Stealth, and at the worst he'll be attacked by one mob. Make sure he never goes off trying to do higher mobs in himself and always put him behind. He works fine. He's also pretty lethal when i'm on a higher level mob and his daggers deal 60 something damage every hit at lightning speed.
Equipment wise, stick him with the Edge if you have it. (If your PC is using it, you're probably better off with someone other than Zev anyway). Otherwise, Crow Daggers are a decent cheap choice. There's also Olaf's Cheese Knife which is easy enough to pick up. For armour, you should be able to pick up some Dalish Equipment for some +dex with little cost.

His starting tactics need work. Make sure he attacks unseen, set him up to attack stunned enemies, use mark of the assassin, etc... and when you get a chance pump his con up a bit, it's only 10 usually.

He's as good as anyone else when set up and geared properly. I had no real issues with him. He would die faster than my rogue but of course I had better gear on my character. He does need a bit of work to make him good though just like any of the other characters.

Just make sure your tank is tanking and not tickling a single mob while Zevran tries to "entertain" three others.
