Achievements and bonus powers in Mass Effect 1, 2 & 3

I'm doing a completely new playthrough of ME1, ME2 and ME3, with an adept. Previously, I've done at least 10 playthroughs each of ME1 & ME2 and one full one of ME3 with my canon Femshep. I'm noticing now that in ME1 some achievements are not unlocked. I had expected the Asari Ally to unlock, as I played through ME1 with Liara on almost every mission, I went to Feros first and took her everywhere after that. Although I may have started some assignments on the Citadel with other characters, I must have finished them with her. Does that mean these don't count? 

In ME2, the opposite is the case. I have the achievements for Zaeed, Miranda, Kasumi and Legion being loyal, even though I am two missions into my first playthrough with this character. I have the first three in my squad, but not loyal. I am nowhere near the part where Shepard meets Legion yet, so why is the achievement unlocked?  Many of the other achievements are unlocked, some of the others are in the process of being unlocked again. It makes no sense to me, seems totally random. Do some achievements carry over, but other's don't? If it is enough to recruit Zaeed to get the 'Zaeed loyal' achievement back if you'd unlocked it previous, then where are the achievements for Garrus, Mordin Solus and Jacob being loyal? Also, I have access to bonus powers for all of the aforementioned four, including Geth Shield boost. I did uninstall and reinstall the game previously, maybe something got screwed up somehow? Could this result in problems later on?

Also, in ME3, I've noticed that I have Reave available as a bonus power, but I don't see how. It should not carry over from ME2 afaik, where I got it from Samara on a previous playthrough. None of my characters have ever let Kaidan live through Virmire, as punishment for him trying to get into my Femshep's pants when all she was doing was talk to him for a bit, until my current playthrough, with which I still need to finish ME2 first. Due to an Origin glitch when a friend used my account, I got their savegames and mine were wiped somehow. They had Kaidan alive in ME3. I've removed their savegames from the save folder, but still see the bonus power available. Did this somehow carry over? And if so, is this permanent? Is there any need for me to keep Kaidan alive, or will all new ME3 characters, imported or otherwise, now have Reave available even if in that playthrough Kaidan has died and been buried long ago? Or can I have him killed on Virmire and won't there be any need any more for me to keep him alive? I really don't like either Ashley or Kaidan, I'll probably end up getting them both killed on my Renegade Femshep playthrough which is up next, but can I do that without risking losing out on the Reave bonus power in ME3?

New achievements seem to unlock more or less correctly now in ME2. Although I've noticed biotic combos sometimes registering and sometimes apparently not registering at all. Despite spamming pull + warp it took a few missions to reach 10/25 of the tactician achievement.

The Ally ones in ME1 are crazy, you need to take a party member virtually everywhere after you get them, so basically once you have Liara she should always be in your squad from then on. It means I needed 3 playthroughs to get all of them.
