PC: Yes, and Windows 7 in case it matters (I've seen others mention this OS).
Additional software: None except 'the usual' (anti-virus, Origin, etc.).

You all need to try the whole looking up fix. Worked for me perfectly.
Sure, it took me 8 minutes of endlessly attempting different things and investigating this thread but I managed to do it when my friend said this:
"Look up when approaching the ladder."
i.e.
"When you're going towards the ladder, look upwards and continue to do so as Shepard begins the 'climbing down' animation."
Skip to 8:30 of the video and see for yourself. Or watch all 8 minutes of me raging and stuff.

• Was the Banshee dead or alive?
• Who was in your party?
• Did you have Adrenaline Rush on/off before/during the ladder climb
• Did you have anything else running on your PC? (several have reponded only game and Origin, so this is less critical)

Only Origins & twice with fraps.

The banshee was dead, annihilated her with the anti-material assault rifle
Garrus and Javik were in my party
I almost always choose infiltrator so Shep doesn't even have Adrenaline rush
I had steam, origin, mass effect 3, firefox, and skype open

For those that have a save game that can reproduce the issue, can you please do the following for me?
- Please send me your system specs, including ram and video card details
- For those with medium to high end PC's can you please try to reproduce the issue with all other programs shut down.
Programs such as FRAPS or other video capture software tend to hog up resources. Having Origin and Steam both open at the same time may also be an issue. Also, if you are running a web browser in the background with several tabs/pages open, please try closing the browser or limiting the number of tabs open at a time. Essentially we want to test how it runs on your machines with only the absolute bare minimum running (game and Origin)
We appreciate your help with this.

that's all the specs i can remember off the top of my head, thanks for the possible solution billy
EDIT: I do not have FRAPS or any other type of screen capture software other than Camtasia Studio 8 and I rarely open that

Desktop: Custom-built PC - Intel Core i7 2600K, 2x Radeon HD 5850s in CrossFire, 8Gb RAM (4x2Gb), 1Tb Seagate HDD.
My wife has completed Leviathan on the XBOX 360 without any issues, so it must be a PC-exclusive issue.

Billy I actually had the ladder glitch on a different level, it was the ladder at 1min 10secs into this video:
no Banshee there
Javik & Liara
Infiltrator with no powers on
ME3, Origin & Kapersky AV running (EDIT: The AV wasnt scanning, it was just on, like always)
AMD Athlon II 640 x4, nVidia 560 EX OC, Win 7 x64 Pro.

If you run into this issue again, we're curious if this occurs if you lower your graphics settings as well.
From what we can tell right now, it's a timing issue with the ladder animation caused by the PC's resources being tied up when trying to play the animation. For those that have found success by looking down or up, this is likely due to the game not having to draw out the full view of the vista in the distance. We'll update as we learn more. Please let me know if you have any success at a lower graphics setting.

Turned off AA and dynamic shadows in ME3 options and so far reduced screen resolution 4 times. Still getting same glitch.
Custom Desktop :
AMD Phenom IIx4 840 Processor 3.20 ghz
4gb DDR3 1333mhz
1TB SATA2 HD
1gb Nvidia GeForce GTS 450
Win7 64bit
EDITED: Just reduced the screen resolution to 800 x 600 (with AA and dynamic shadows on) and there was NO GLITCH!!!! It failed for me at every attempt above this resolution with or without AA and shadows turned on/off.

I'm only running Origin in the back ground - no steam or web browser etc and I still have this issue on my Win 7 pc
