ASUS K42Ja-VX062 (i5-460M @2.53GHz, Radeon 5730M, 4GBx2 RAM)
Apps:
Fraps running, with MSE and Origin 9 with disabled igo32.dll
FXAA Mod running @ SMAA mode, disabled bloom.
Settings:
1280x720 windowed mode, with all settings enabled and set to high, save for spherical lighting(?)
Spherical lighting set at 2.
Frame rate holding at 30-45 during the portion, with lowest jumps at around 23-24.
CPU usage for ME3 ran at 20-40% during gameplay.

Party: Garrus, Liara
Banshee: Dead
class: Infiltrator (Adrenaline Rush disabled)
Additional Notes:
Happens only when camera focus is placed for a while on the ladder. If ladder sequence was rushed, glitch happens lower.

I haven't installed the Leviathan DLC yet and have this problem.
Got stuck on this ladder (second ladder going down after entering the Monastery) once before in my first or second play-through (ME3 Extended Cut), and had to restart the mission. Last night I got semi-stuck on the ladder again, but was able get loose after getting stuck several times.
I got down this ladder by looking up, not all the way up but, about a 45 degree angle up.

Running ME3 at 1280 x 720 resolution. It runs smoothly without any loss of FPS during battles...
MS Security Essentials
Nothing else running in background other than Origin.
One thing I noticed since the newest update to Origin is that in Offline and Online modes, every time I start the game, the Store/My Games Origin window opens now instead of just the gadget on the Win 7 menu bar. This is a waste of system resources and could cause performance issues on PCs that are older or have limited Ram. There is no option to stop the Origin window from opening on start-up of the game.

Team - I've been through that ladder several times now, with a team usually consisting of Vakarian/Alenko, Alenko/Vega or Vakarian/T'Soni. The Banshee is always dead before I attempt it.

edit: I am running the PC version, but nope, I wasn't running anything in the background when this first occurred. Now i am only running Firefox just because I am trying to find a fix on this forum.

As a bonus, you'll also no longer have to quicksave before talking to Joker, because if you get caught in the cockpit geometry you can just ghost out of it.

Yep, I turned my resolution down to 800x600, looked up going down the ladder, and I got it on the second try.

phew, I am so glad to be able to find a "solution" to continue with the game. bioware needs to fix this bug. This DLC is so great!!!!!! I am glad to be able to find how the reapers actually started. They got a beginning and the child star is just a creation. I hope to see more DLCs like this one.

The way it's described here, I have experienced in action on a multiplayer map. The Vancouver rooftop has an awful ladder that bugs constantly. From the spawn point, run forward and hang a right at the second set of air ducts. You will see a platform above with ramps going up either side. The ladder is in the middle. Nobody will go down that ladder willingly though, since the character is frequently hung up on it, right at the top - a big meat target when you need to be moving fast. Everyone uses the ramps, or dives off the ledge to either side of the ladder.
But no one ever pops back up it when trying to go down.

Just downloaded the Leviathan DLC day before yesterday, I went down this ladder last night with no problems, approached it looking down at it and Shepard mounted and slid down the ladder without issue.
On the first mission of Leviathan I did get stuck on that ladder, before the Namakli mission but, only for a few moments and then Shepard fell off.
I'm on my 6th playthrough and I've seen this issue before, at the beginning of the Asari Monastary mission and in the Geth ship, and since have approached all ladders with caution.

I had no success with looking up or moving the mouse around. Maybe I didn't do those right. The only thing that worked for me was changing the resolution to 800x600. Once i was down the ladder, I changed it back (1280x720) and didn't have any more trouble during the rest of the game. This is the only time I've ever come across this problem.
To answer the above questions:
-The banshee was dead.
-I tried once with EDI/Javik and once with EDI/Liara.
-Didn't have Adrenaline Rush.
-Only the game and Origin were running.
