Mine would have to either be auto-dialogue or lack luster side missions.
What is Mass Effect 3's biggest flaw to you?

Casper the star ****** & his gibberish, every thing else pales into insignificance compared to that.

Autodialogue, the endings and the horrible treatment of ME2 LI's. (Minus Garrus, of course)

Getting rid of the interesting ME2 squadmates, and just using Tali and Garrus from ME2.

All of ME3's flaws can be summarized with just two words from the esteemed Mr Hudson:
"... too videogamey."

As much as I like the whole game EC ending included, I think more could've been done with Earth and Citadel.

The cameo appearance of Harbinger.
Seriously, no conversation or anything after all the build up in ME2 just hugely disappointed me.

-No confrontation with Harbinger.
- Revelation of Reapers origins. (truly, how can a writer who considers himself a good one be unable to understand what made Reapers such an iconic scary enemy. By revealing the truth about them they automatically diminished the threat. No matter if it's a well thought out story or not. Which is not, by the way.)
-No continuation of HEASTROM subplot which was strongly hinted as a major one in ME2.
Everything pales in comparison and would be acceptable as also ME1 and ME2 have their own problems. Nothing to serious for those who love the series though.

Major decisions you've made throughout all 3 games do not affect the ending.
