Biggest? Lack of choices...the conversations lacked depth in choices, the endings lacked my choice (why can't I choose to get all the assests and a high EMS to survive - without headcanon), no choice in hub world exploration - just the Normandy and the Citadel? Really?! Could go on about this, but why?
I wanted to love ME3, but it was just lacking for me.
What is Mass Effect 3's biggest flaw to you?

Everything after transporting up to the Citadel. I've spent a total of 600 hours in ME 1 and 2, building characters and enjoying different characters and interactions. I felt completely betrayed by the ending and refuse to let "my" characters suffer through it. I won't buy any more Bioware games until they give me a positive ending for ME 3 that preserves the ME universe and lets your characters survive if you are 100% on Paragon. Eliminated the horrible plot holes would be a plus!!!!!

Priority: Earth being a generic Gears of Wars mission.

Yeah. I wonder how all this people asking/suggesting/begging new DLCs felt when they saw this message at the first time.

Probably the Rachni mission. I saved her anyways, so I didn't notice at first, but then I heard about what actually happened... What was the point of giving us the choice if you were just going to sideline it like that.

The ending, I expected the crucible to be some big ass Deathstar thing that vaporizes reapers by looking at them, and all I got was...a kid

No side quests (eavesdropping on people, scanning a system, and getting a number is not a side quest).
Everything that is optional is a side quest. So missions like Grissom Academy, Aralakh are all side quests.

That kid's voice. I heard it and wanted to burn him to the ground. That, and I wish there were more things involving love interests, even if it's just more dialogue. Also, the ending. Not for the same reason as everyone else, but because I hate having it revert to before Cronos Station. I loved having a 'free galaxy' to explore, like in Mass Effect 2. Other than that, I loved the game.

The rail shooter sections. There was a lot of potential in them, but they were so short and offered so little and in the case of the reaper on Rannoch you didn't even need to shoot the gun. It all felt like unfinished filler that left me thinking "why?"

The journal.
Honestly that is my biggest legitimate grievance with the game now with the extended cut and new DLC. It would've been a lack of Harbinger dialogue, but Leviathan made up for that.
I just wish Sovereign's theme played when conversing with the Leviathan, I feel that theme was so underused in ME3, it would've been a nice touch.

Endings and fetch quests where you CANNOT land (let me explore some homeworlds...)
Edit: and Harbinger's lack of death threats to Shepard.
