Change, definitely. Having a refugee running around with a small child in a warzone filled with Reapers who want to kill you and goo you leaves alot of implications as to how mother and child got there in the first place. It'll take some pretty slick writing and scen setting to set that up.

Well, if we went with the idea of a mission hub at the base we could introduce a mission where Anderson directs Shepard to evacuate refugee's he's stashed in a zone that's going to become a battleground. Add reports of collector esque Reapers having been spotted in that area and you have a made for Javik mission. Add some dialogue about how the rest of your squad has had to head off on other missions it could be a Shep and Javik solo act. This would give alot of scope story wise for the player to accept Javik's view in life, i.e. be heartless and focus on the mission regardless of the cost, or challange him to overcome his rage. Sorta similar in the way Zaeed had to go kill the bad guy or save the civvies.
I don't think a shep + javik act would work since his dlc is self-contained. People would be annoyed at missing out on a whole mission if they decide they don't want to buy from ashes.
However, the base idea works. What if there was a mission where you can only take one crew member with you, and they all react differently to the citizens? Thing is i don't know that any of them wouldn't go to save them...
Taking the base mission hub idea forward a bit. How about having a mission for each squad member? Garrus could go with Shep in a mission that focuses on seeing how the Turians fight. Tali could go with Shep to see how Quarians deal with the assault. Edi could be the Geth viewpoint.
One of the things I like about squadmates is that they bring their own viewpoints with them. When Geth talk about the consensus and how their intelligence dims I just nodded and went with it. It wasn't till I ended up inside the consensus and shut it down that I saw what Legion was getting at. These mini adventures can be roller coaster rides. But making them something where you come away a little wiser as to who your squad mate is, is one of the marks of why ME has been so successful. Vega definitely needs a treatment like this as he has to get over losing his ppl and choose his future in the military.
I think that if we start to add in actual gameplay to the mission, the dlc would get way too big. Bear in mind that this would take 7 whole new missions, in addition to everything else.
Maybe one or two new missions, but 7 is probably asking for too much
