I've spent somewhere around 4000 hours in-game for World of Warcraft. On the other hand, I've played thousands of Rock Band songs.

Besides those two, the closest runner-ups would be Mass Effect and Megaman X, in the order of 10-15.

*edit* Skyrim with around 746 hrs of playtime combined for all my characters.

2nd place goes to Metal Gear Online with 500+ hrs of accumulated playtime.

Not sure which is my most played game but CiV V + expansion have over 1600 hours.

Fallout New Vegas: Multiple 50hr plus playthroughs, around a dozen or so I think.

It's a marginal victory though since I've played Pokemon games, Mass Effect, Fallout 3, Oblivion, Skyrim and Dragon Age Origins for easily 300hr+ periods.

Wow. I thought I was bad with about 80. 

For me, the game I've spent the most hours on would probably be Secret of Mana - I'd play it, beat it, and immediately restart it. Too bad it doesn't display your playtime. Or actually, maybe that's a good thing, because I'd probably be horrified at the amount of time I spent on it.

Dragon Age Origins is at 6 or 7 playthroughs I think.
Mass Effect 1 and 2 had around 7....
The old Megaman series 1-7 and the new ones 9 and 10 are pretty high (I'm not even going to guess)
Megaman X 1-3 have at least 15 or 20 among them each (still play them on occasion)
Batman Arkham Asylum easily has 10 playthroughs on its own.
But Arkham City probably has 11 or 12 and thats been out barely a year now... Hell I'm on a new game plus playthrough right now!
