Not counting that it's either Mass Effect 1, KotOR 1&2, Max Payne 1 or Yoshi's Island for total hours spent. Monkey Island 1&2, Indiana Jones and the Fate of Atlantis, Day of the Tentacle and Full Throttle for completed playthroughs.

Games in general? Command and conquer 64 ooooor GTA Vice City considering I killed 1 million people.
In recent time all runs counted I have at least around 500 hours in Skyrim.

Yep... WoW is the game I spent the most time playing.
Then again it's hard to say how much of that time was actually spent playing. I loved just hanging out chatting with people. The game wasn't all that great without that part.

I have over 30+ playthroughs of ME2
500+ hours in Skyrim
Lord of the Rings Online over 3 years played
Neverwinter Nights hordes of the underdark not sure how much time spent with this game but it would be a ton.
Final Fantasy 10 - 500+ hours easy
Final Fantasy 12 - 500+ hours
Final Fantasy 13 and 13-2 almost 300 hours last time I checked.

More recently, it's probably Icewind Dale at nine times fully through, followed by both Baldur's Gate games with six times fully through and Icewind Dale II with five, then Dragon Age: Origins with four and Morrowind with two. All of those have several more that I didn't complete for whatever reason also. I know that I played both Knights of the Old Republic and Knights of the Old Republic II several times as well, but I don't remember how many times - I don't still have those saved games, for whatever reason.

Of course I do work on the Homeworld 2 complex mod http://www.homeworld2complex.com/
I still spend time in Morrowind then any other TES game and as far as BioWare games go... It would be a toss up between DA:O, DA2, and ME2.

I've enjoyed Civ 1, 4 and 5 quite a bit as well. With Civ 5 for the first time, I've played a lot of short matches with friends on Steam. However, for the single player campaign I always go for the huge world, marathon pace, and a single game lasts months or even as long as a year or so. Not counting MP matches, I've only done 2 or 3 playthroughs since I bought the game at launch.
I've played five different characters for Skyrim so far and just started another to try Dawnguard. I join only one or two factions with each playthrough and each seems to last about 100-200 hours and then I usually start over around level 20-30 to try a different group of mods with a fresh character. Played close to 800 hours in total and still haven't completed most of the faction questlines yet or even started Dark Brotherhood.
I'm guessing I played Oblivion around the same number of hours, but rather than making multiple characters in that game with different playing styles, I tended to play the same kind of jack of all trades battle mage / stealth character each time, with some really long playthroughs and only a couple of different characters.
I played DA:O a couple hundred hours and made multiple characters - really enjoyed the different origin background prologue areas. Played DA2 about 250 hours with 2 characters and although it had some flaws, overall I really enjoyed the game.
Enjoyed ME3 the most out of the three Mass Effect games. Not sure exactly how long I've played as Origin doesn't log your playing time, but I'd say it was around 80-100 hours for one playthrough. Will probably try a second at some point after all the DLC is released.
As for the highest total number of playthroughs, I'm sure I played Super Mario Bros. on the 8-bit NES hundreds of times, only actually completed it once or twice IIRC, although I was really proud that I made it to the "Minus World."

It's odd. My backlog of unplayed games is immense, but I still periodically do a run through the old Infinity Engine games from time to time (currently running a 3 character party through IWD yet again) And since I just received ME3 as a gift, I'm thinking of doing a run right from ME1 through to the end soon.
*edit* Oh, time played? Well that's easy:
Neverwinter Nights by far the most. Hundreds of hours on that. Dragon Age:Origins and Skyrim both have close to 200 hours total clocked on them and then there are the infinity engine games with a combined total through all games of what must be several hundred hours as well. Many, many others probalby clock in at close to 100 hours.

I accrued about 168 hours (a full week) in the first month.
I think I've played through about 15 or so times. Yet to beat that one.
As for ingame hours, the ME series all together would be the first, followed by DA:O at sometime around 200+ h then I think Skyrim (100+). I think.
Actually, maybe maybe that's trumped by Simcity 4, though I have absolutely no clue how many hours I've played. Had it for ten years though so it would be a lot, lot.
