A funny thing happened.

I started playing Leviathan, got the first achievement all good, saved then quit. Then I logged back in to finish it, and I was somewhat curious to see that no achievements were popping up. The ones related to generic stuff (ie Veteran/Gunsmith) kept updating regularly but none of the story-related ones were appearing.

Moreover, once I finished the -excellent- DLC, I was curious and went to check. Not only were they all locked, I had also lost the one I got during my previous hour of play!

All the while I never disconnected from Origin or the game servers, so any help would be greatly appreciated!

It seems that the notification only ever shows up for the first one, and a lot of times they appear locked in the accomplishments list even after they've been unlocked.

They are likely all unlocked already for you if you played to the end of the DLC. Usually if you load a game and play around for a bit and then exit back to the main menu, they will correctly show up (but if you quit and relaunch the game, they will appear locked again).

(Sometimes, the From Ashes accomplishments now disappear as well.)

Yeah noticed that myself, really annoying that it keeps doing that, it's kinda like what happened with From Ashes accomplishmenent's they're up and down like a yo yo, hopefully this will get patched soon -- note my sarcasm

I've just noticed the same thing - I got all the Leviathan accomplishments on the first run-through, now they are showing as locked in the menu.

I also have a bug where the overload accomplishment is only tracking in MP not SP as the other similar ones did - most annoying.

never encountered this on From Ashes but just lost all achievements for Leviathan like said above.
