I had a look through the first few pages and didn't see this topic, so I thought I'd make one.

I just completed my Insanity Playthrough (Completed all N7, Hub and Side
missions alongside all Priority ones) and the Insanity achievement
didn't pop at all. When I started this playthrough, I set the difficulty
to Insanity when importing my Shepard and I didn't change it at all
throught the playthrough. I do have the EC installed (I chose the Green
ending this time) as well as Firefight DLC, but I would think that they
wouldn't of interfered in the unlocking of this achievement...

So,
is the Insanity achievement glitched or what? I deliberately followed
the walkthrough in the official guide, to ensure I did everything in the
game, so I'm just wondering what happened? Do I have to do another Insanity playthrough to unlock it? (Something I don't really want to do to be honest...) Any help/advice would be appreciated.
