ME2 Insanity was easy as heck with Soldier. Vanguard builds for Insanity?

Start by getting Heavy Charge. Then you can either get inferno ammo or, and personally I prefer it, get the Guardian passive. Then you get the other of the two previously mentioned you did not get. Slam is the best bonus power unless you insist on leaving Miranda and Thane out (they set off the warp bombs slam sets up). The Claymore is the obvious choice if you don't mind the reload trick (pressing the melee button or activating a power in the middle of reloading while holding the fire button to cancel the reload animation and force a shot). If you don't do the reload trick it's better to stick with another shotgun (eviscerator, scimitar and geth being the useful ones) and you can train an additional weapon instead.

The rest is immaterial. Personally I prefer not getting squad Cryo Ammo because freezing is for a Vanguard detrimental almost as often as it is positive. I just hate it when they freeze an enemy my puller has flying and send it crashing to the ground with the option to charge them off the map or blow them up gone. If you like to bring non biotic squadmates cryo ammo is less likely to annoy and disturb your plans. I also prefer one point slam to higher level slam because they don't fly as high and land faster which is positive if you need a shot to finish them off which you will most of the time. This leaves me with enough points to get area pull but I rarely use it.

Heavy Charge+Inferno Ammo is the unique aspect of the class and then the passive to boost that (you can get Destroyer instead of Champion if you want to really max your damage but I prefer the shorter cooldown). Anyway the last 21 points you can do what ever you want with. I max shockwave after area pull it's good for a laugh every now and then. Not by any stretch of the imagination good. You could skip those and max cryo (bleh) and slam or some other bonus power instead.

Have you checked the List of Strategy Guides Found on the Forum thread listed at the top of the Mass Effect 1 & 2 classes, Strategy and Builds Forum?

Basically what Gallimatia stated:
1) Max Charge to Heavy Charge
2) Max Combat Master to Champion or Destroyer (does not really matter a whole lot)

And you want to continue to play as a Charge-heavy Vanguard fter that, most people do this:
3) Max Incendiary Ammo to Inferno Ammo
4) Max Cryo Ammo to Squad Cryo Ammo
5) Max Shockwave to Improved Shockwave  -OR-  max your bonus power

If you want to cast more biotics rather than charge all the time, you can do this:
4) Put points into Shockwave to unlock Pull
5) Max Pull to Area Pull
6) Max Shockwave to Improved Shockwave
7) Max Incendiary Ammo to Inferno Ammo

For bonus powers, Slam is good for setting up warp bombs with Miranda/Thane. Stasis is also good, particularly if you want to exploit the damage bug.

Claymore is a very popular choice for weapons training. Sniper rifles are also decent.
Unless you want to exploit the time dilation of Heavy Charge with the Mattock, do not bother with Assault Rifle training. Not worth it on the Vanguard.

I also think Slam is about the best bonus power to start with early on a Vanguard. Later on it is debatable, with Stasis probably being a little bit better. But before you get your core powers leveled Slam has a quick cooldown, works at 1 pt for CC, and sets up Warp Bombs.

Weapon training, when you get there, is debatable. I think taking Sniper Rifles and using the Viper increases your versatility dramatically, but some people rather play to the strength of the character and go Claymore since it is ridiculous with reload canceling. Either way works. I wouldn't bother with Assault Rifles training unless you have the Mattock, or really just like ARs. I modded the character to do a game with just AR's and Pistols on the Vanguard (AR Vanguard) though :)
