In the Accomplishments menu, although it is not a Tophy, I have the Unwavering Accomplishment seemingly stuck too. I'm up to Priority: Cerberus Headquarters, in game on Insanity & have completed all of the Galaxy at War side missions & both DLC packs, yet Unwavering is stuck at showing only 4/27 Single Player Missions on Insanity.
I am unwilling to continue playing this game as I do not believe it will award me the Trophy for having completed the game on Insanity, I have already seen other people on the official BioWare forum commenting that they have not been awarded the Insanity trophy, despite completing the game on that level.

The Freedom Fighter Accomplishment has not awarded, despite me gathering all of the intel on Eden Prime.
The Prothean Expert Accomplishment has not awarded.

I'm not sure where the No Stone Unturned, Family Matters, Under Pressure & Conspiracy Theorist Accomplishments are supposed to be gained, but I have done absolutely everything in the game up to this point (Priority: Cerberus Headquarters) and have not gained any of these either.

This seems directly related to my other issue regarding the Explorer & Tour of Duty trophies not awarding as the in game Accomplishment counter has got stuck.
