I'd say its cause when it comes to actaul rival characters.
BW kinda sucks at it.
They tend to make Heroes massive player empowerment fantasies that noone in universe can take in a fair fight-unless your villian is a borderline god... and even then it could have gone either way-
With Leng, They never really give him a personality... His just thier to give Cerberus troops an actaul face cause fighting TIM would be wierd. IE Leng wasn't brought in to be a character, His just the Cerberus Avatar-which says a whole lot about Cerberus in ME3-
So they resort to the most basic insults and Cerberus spins they can think of.
with Shepard... You'd think with Shepard clearly being more scriopted by the writers then the players BW would give us an interesting Protagonist. Shepard is so freaking bland it hurts
They clearly have a default alignment in mind so it's jarring when you do something extreme paragon or renegade. and let's be honest Default Shepard is the most incompentent boob in the universe.
so you'd think with a scripted Shepard, they could come up with better banter then essential two full grown adults argueing with 2nd grader insults.

Unless bioware forces a person to die (Virmire) or script it so Shepard is momentarily defeated (Thessia), then Shepard would go through all 3 games as a infallible, unbeatable God-like character. Lets say it was possible for shepard to win and lose on Thessia, who in their right mind would go for the option where they lose? Its the same thing with the suicide mission, of course Shepard CAN die, but to do so you really need to cut off your nose to spite your face. Its pointless.
I'm in favor of Bioware having moments like Virmire where Shepard can do all he can yet still its not enough/he can't save everyone. I think the suicide mission would've worked better that way, granted it felt epic getting everyone out alive, but I can see from a gameplay mechanic it really didn't pan out the way bioware intended.
The biggest problem with the whole Kai Leng business is why Shepard took it so personally/to heart. At least for renegades it seemed out of character.

I think it's because Mac Walters wrote him, because as OP said, whenever Kai and Shepard are in the same room, they both come off as lame man-children.

Personally I have no issue with Shepard "losing" on Thessia but the way Shepard losses was a bit odd. You beat up Kai Leng and then a gunships shoots pillars so the roof falls on you. Erm...
In ME1 you have a fight with Saren where you punch him in the face and he runs off and it doesn't feel so bad...

I hate that this **** brings a sword to a gun fight. I don't care how space ninja-ish you are, that doesn't make any sense.
In Cowboy Bebop I found it a bit contrived, but it was Anime, so I let it slide.
Mass Effect is supposed to be, somewhat, grounded and then they introduce this element?

It's one thing if gameplay would also show that he is troublesome to hit and thus make the sword a somewhat smarter thing to use. However, he never got near me on Thessia and he still wins?! WTF? That's pants-on-head retarded.

The only] reason(s) I never liked Kai Leng's sword is because he keeps saying "We evolve or we die! THOSE ARE THE OPTIONS!" while fighting me with a sword....
Also making him a ninja bugged me but that's not the sword's fault
