Did this quest on my first playthrough.
Now im totally lost as to where i got the notes - need help.
Notes i always get are the werewolf lair note (many traps in the room) and the note in the temple of the urn of sacred ashes.
What/where am i missing to get this quest?

Last section: talk to the frightened dwarf adventurer in the tavern in Orammar. He'll give you the final note that opens up the last step of the quest.
