I started a new game last week and after the character creation, I went to the Options in the Main Menu and set the difficulty for Insanity and upon completion of that game, no achievement for Insanity popped. This is the second time I've finished this game on Insanity, and according to the achievement description, to earn this you:
Finish the game on Insanity without changing difficulty after leaving Earth.
Like I've said, I've done this twice... even completing all side missions etc (even though the description itself gives one the impression that you just need to play the priority (story) missions and complete it... nothing implied about the extras) and they all end the same way... No achievement for Insanity earned.
I know this may seem a somewhat trivial query, but as I recently earned the last achievement I needed for 100% in Dragon Age: Origins (Grim Reaper), I want to get this game 100% as well... until if another SP DLC comes out that is. Any information/advice would be appreciated as I feel that the last two Insanity Playthroughs were just a waste of my time, if the achievement never popped...
Is there a glitch for the Insanity Achievement?

I've never heard of this before, but I'd really like an answer too. Don''t want to start an Insanity run if it won't count.

I got that achievement with no issues (first play through) got almost every achievement, except for the one for getting the majority of the EMS for the final fight, and getting a weapon to level 10, as I played straight through the SP before I even touched the MP. You may have had the glitch that resets your Achievement Progress, which is related to this glitch. hope this post helped.
