Don't use consumables on bronze or silver unless it's wave 10 objective.
Will use rockets starting from wave 8 in tight maps like Glacier and/or spawn points. Always whip out rocket launcher on wave 10 even before I know what the objective is....

When a match starts, I pull out my launcher and fire all five off immediately. That way no one can ask me for a missile later.

I use them on objective waves.
When Banshees get behind the defenses.
When the end of the mission arrives I shoot them all at the planets around me just in case there is a Reaper on one.

I have over 200 missiles so my rules aren't strict. I play gold and typically use them for these situations:
1. See teammate about to be sync killed by a Banshee. My timing has gotten better.
2. Past wave 6, want revive someone and they're covered in bosses I can't kill in time.
3. Stuck in the revive cycle of doom and the only way out is to blast the ground.
4. Wave 10 objective I let 'em loose, no matter the task. I typically wield the missile launcher the whole wave.

Here are my rules:
1) Killing 4 key targets. ONLY on Wave 10 and only against boss units because you want to guarantee those credits.
1a) Using a missile on a key Phantom is acceptable.
2) Multiple boss units (2 or more) all sharing the same 3 meters of radius.
2a) If all boss units are near full health, fire.
2b) If one of the boss units has lost all their shields and half of their armor, do not fire unless the situation is urgent.
3) If you need to clear out a spawn from a hack/upload/pizza/drone objective.
4) NO GLITCHING
5) If the last one standing and being chased by two or more Banshees in the same radius, fire.
6) If there are three or more Phantoms sharing the same three metre radius, fire.
7) Try to aim at the feet if possible. You can aim for the body of an Atlas, but this is not preferred. They have magical powers that can read your movements and trajectory to get out of the way at the last second.

when my team is dead, and I have no medigels left: I pull the missile launcher out, aim it at my head, and pull the trigger. That is the only rule

Hack = at least 1 rocket (unless I am playing with a Chuck norris type)
