Looking at the screenshot, I think the story it tells is that Iron Bull farted and it hit Viviene first.

Am I the only one who thinks that Iron Bull looks kinda short? When comparing him to how the Arishok towered over the humans in DA2, IB really doesn't seem to be that tall.

It's hard to tell. It looks like he's standing a little ways back from the rest of them in that screenshot.

Yeah. He's quite a bit further back than it appears in the screenshot. 

 

You can look at the Inquisitor for an example, if he were to stand up in that position he'd tower over Iron Bull. I think Iron Bull is pretty big, you can see him on a more even perspective in the latest trailer at the end.

Sadly, a blow to the head from a demon has bent it, leaving the helmet permanently affixed to his head.
