This makes me wonder if the roles of the party members are randomized in cutscenes. For example, in that Venatori scene, two of your current members go to hold the gate and the last companion remaining stays with you.

Doesn't mean she's dead though - they'd hardly show a companion death in a demo reel months before the game launches.

I wonder if its a case of having to choose one companion over another as you advance the story. If you can end the game with only 1, then it's quite possible.
