I seriously think the Inquisitor, or at least if not a human, was simply caught in the blast radius. The blast had to be huge to kill the thousands that Cassandra says it does.

This is why I think you might just be passing through the area where the Breach happens.  Which is sad but makes the most sense from what I know of the story so far.

I give it a fifty fifty chance, and really maybe only in a cutscene or two.  I doubt the Inquisitor is going to be at the actual Temple when the Breach occurs interacting with people. 

 

And I just had a horrible thought.  What if the Dalish Inquisitor is traveling with his/her clan through the blast radius so when it happens their entire clan is wiped out and its just them.

Yea, it might just be a cutscene

 

Though I would much prefer a Ostagar type opening for you to get set up what type of character you are and meet some of the characters there

Well, we lost our friend and were forced to leave our clan in DAO. Bioware has of up the despair .
On mage leaders dying, I guess my theory of Rhys dying in the start of the game, leading Cole to revenge him and join the Inquisition won't be welcomed here.
