If it's the gameinformer article, he said he saw a character die that he wasn't expecting, and that many fans might not even see that wasn't necessarily the Sera scene. I'm still very concerned for my characters, savescum away

Same. Will do everything I can to make sure such a thing does not come to pass. No companion (or advisor) deaths on my watch, if at all avoidable.
Though I am still not convinced. It seems an extremely anti-climatic and strange way to kill a companion, without any player control what-so-ever.

I think that's the snowy area with the Red Templar fortress, whatever it was called. They've said that whole map is a battlefield, and the main plot arc involves gradually pushing them back and establishing Inquisition control until you reach the main enemy keep.
People complained so much Leliana was "anti-mage" in DA2 that Bioware decided to actually give them a reason to.
